Enrollment Process
 
Enrollment for the School District of Menomonee Falls is processed at the Administrative Offices.  Which are located at W156 N8480 Pilgrim Road, Menomonee Falls, WI 53051 on the 2nd floor in the Village Hall.  The offices are open 8:00 am to 4:00 pm, Monday through Friday. 

We require the SDMF Enrollment Form filled out along with two of the five items listed below as proof of residency. 

  1. a current WE Energy bill in your name with current address (if you pay on-line, bring a print out)
  2. signed current lease with all parties listed (if registering for fall 2013 your lease must be current and through Dec. 2013)
  3. signed current closing papers in your name
  4. driver's license with current address
  5. state ID with current address

Your student will be given a packet of information for the school they will attend. 

If your child is entering 4 year old or 5 year old kindergarten for the first time, a copy of their birth certificate, baptismal certificate, passport, or affidavit is needed to register your child.  Please call 262-255-8440 if you have any questions.

To be eligible for 4 year old or 5 year old kindergarten in the State of Wisconsin, a child must be four or five years of age respectively on or before September 1st of the current school year. Proof of age must be submitted by presenting one of the following: birth certificate, baptismal certificate, passport, or affidavit at the time registration.
 
Parents whose children become 5 years of age after September 1st of the current school year may apply for early admission to 5 year old kindergarten. A written parental request for consideration of early admission should be made to the principal of the elementary school where the child would attend. This request should state specific reasons for consideration for exception to present entrance dates. After review of the written request, the principal will contact the parents explaining the procedures involved.

Receiving and agreeing to reading the handbook is a requirment.  
You will be asked to sign-off that your have received & will read the handbook on Registration Form.

                         

Tuition Waiver
  
What happens if you are a resident and your child is currently attending the School District of Menomonee Falls but you move out of the district?  If your child qualifies they may continue to attend until the end of the school year by completing and submitting a Tuition Waiver form to Jayne Paddock.  A Tuition Waiver is a state program and monitored by the WI Department of Pupil Instruction.  Qualification is determine by answering the questions on the Tuition Waiver form. Transportation is not included for Tuition Waiver students.
